Durable Power of Attorney (Effective upon Disability) – Allows an individual (called the “Principal” or “Grantor”) to authorize someone else (called the “Attorney-in-fact” or “Agent”) to act on his or her behalf. This Durable Power of Attorney document becomes effective upon the disability or incapacity of the Grantor.
